Digital Forms on PowerSchool ~ Step by Step Instructions for Parents

Parents and Guardians,

    The district has asked that every Parent or Guardian update their Contact information, AUP form, Parent Release and Authorization form so that it is all digitized. By taking a few minutes now to do this, you will not have to sign these forms again unless changes are made to the forms by the District or if your child changes schools!  

*This will be a huge time saver for the school and to you in the future, especially at back to school time next year! Please take a minute and follow these steps below! Thank You!

Go To:   https://powerschool.ccsdut.org/public/home.html

Log in - this needs to be done in your parent account.  (not student account)

    • If you do not have a parent account, please create one first at: https://powerschool.ccsdut.org/public/
    • Click on second tab “Create Account”
    • If you need help creating a parent account, please access the video tutorial located on the login page of Powerschool:  https://powerschool.ccsdut.org
    • After creating your account - link your students to your account. Click here to see how this is done. http://www.d91.k12.id.us/documents/SSOWorksheet.pdf
    • YOU SHOULD HAVE RECEIVED AN E-MAIL FROM CACHE COUNTY SCHOOL DISTRICT WITH INFORMATION ON HOW TO OBTAIN YOUR STUDENT’S USERNAMES AND PASSWORDS. You will need this info in order to link your student’s accounts to your account! If you did not get an e-mail with this information - PLEASE call River Heights Elementary and ask for this information for your students. 435-753-4948
    • Now you are ready to sign the digital forms! * You will not have to sign these forms again until your student changes schools, or forms are updated.

Once you are on your account on PowerSchool:

  • Click on Ecollect Forms - link  (Under Navigation on the Left hand side of the screen about halfway down)
  • Please click on CCSD Update 1 - Contact information Tab
    • Note: there is a ^ to the right of the submit button. If you have other students, click on the ^  then click on “ Submit for Family” at the top of that pop up menu. It will show you additional student names checked. Click on submit. Then it will show green check marks on the right of student’s names - Click CLOSE, then SUBMIT at the bottom of the page.
    • Please click Submit in the bottom right hand of your screen when you have completed this form.*
    • You should get a thank you and click ok
  • Please click on CCSD Update 2 - Student & Parent Addresses information Tab (The tabs are now at the top of the page - you will be asked if you want to navigate away from that page - click OK)
    • Please click submit in the bottom right hand of your screen when you have filled out the information - or have checked the information that is already there for accuracy. * Remember to click the ^ so it will update all students in the family.
    • You should get a thank you and click ok
  • Please click on CCSD Update 3 - Emergency Contact information Tab ( top of screen third tab)
    • Click ^ to right of submit button to add this information to all of your students “ Submit for Family” Submit / Close / Submit
    • Please make any needed changes to your contact people
    • Please click submit in the bottom right hand of your screen when you have completed this form.
    • You should get a thank you and click ok
  • Please click on CCSD Update 4 - Acceptable Use Policy Tab ( Fourth tab)

* This has recently been updated. Please read it through.

    • Please click submit in the bottom right hand of your screen when you have completed this form.
      • Click ^ to right of submit button to add this information to all of your students “ Submit for Family” Submit / Close / Submit
      • You should get a thank you and click ok
  • Please click on CCSD Update 5 - Parent Release and Authorization Tab (last Tab on right)
    • Click ^ to right of submit button to add this information to all of your students “ Submit for Family” Submit / Close / Submit
    • Please click submit in the bottom right hand of your screen when you have completed this form.
    • You should get a thank you and click ok

When you are done:

  • Write down your username & password for future reference!
  • Sign Out - top right corner.
