Youth in Care (YIC) Program
Cache County School District Youth in Care provides education services and interventions for individuals K-12 who are in custody of the Utah Department of Health and Human Services or an equivalent Native American tribe.
Submit the following information to Angie Sorensen (angie.sorensen@ccsdut.org) for school placement:
- Youth in Care - Required Intake Information form
- Entrance Application - (7th-12th grade)
- Current copy of IEP (if applicable)
- Current copy of transcript (9th-12th grade)
Once placement is determined, the caseworker/case manager will receive a Permit to Register, which will allow the foster/proctor parent to begin the online registration process.
In order to register, the foster/proctor parent will need to provide the school with
- Permit to Register
- Birth Certificate
- Immunization Records
