Home and Hospital

The Home & Hospital program is designed for students with a medical need that prevents the student from attending in-person school to take the time necessary to recover from their illness or injury. School contacts and educational services are provided to meet the academic needs of each student in the program. Home & Hospital services provide students with resources and tutoring to complete their school work and earn credit while learning from home. 

The primary purpose of the Home & Hospital program is to help students return to school as soon as possible, giving them the opportunity to benefit from their school experience and receive a hands-on education.

Home and Hospital Categories:
  • Short-Term H&H - Health Professional Statement of Needs states that the student needs to be out of school for a minimum of ten consecutive days to forty-five days.
  • Long-Term H&H – Health Professional Statement of Needs states that the student needs to be out of school for a minimum of forty-six consecutive school days or longer.
Home and Hospital Tutoring:

Tutoring is available to any student in the Home & Hospital program. Home & Hospital tutoring is an option, but is not required if a student does not need or want to receive these services. The Cache County School District Home & Hospital team may take up to 10 school-days after receiving the completed Health Professional's Statement of Needs to secure a tutor for students that qualify for Home & Hospital services and choose to receive help from a tutor. During those 10 school-days, it is the school's responsibility to provide the necessary materials and instruction to the student(s). 

Home & Hospital tutors provide up to two hours of weekly virtual tutoring for their assigned student. If a secondary Home & Hospital student needs additional help in math, beyond what their assigned tutor can provide, the Home & Hospital team has a designated math tutor that is available on Mondays and Thursdays via Zoom to provide math tutoring to any secondary Home & Hospital student. HOME AND HOSPITAL REGISTRATION

School administrators or counselors must register their student(s) for the Home & Hospital program by submitting an enrollment application.

A "Health Professional's Statement of Needs" (listed below under "Forms") must also be completed by the student's healthcare provider and submitted to a member of the Home & Hospital team via email or fax. This form must be submitted before enrollment for the student can be completed. Details for completion and submission are listed on the form.
