Policy No. 3270: Catheterization for Students in Schools
It is the policy of the Cache County School District that trained and designated school personnel may perform clean intermittent catheterization (CIC) or assist students with self-catheterization in the school setting when ordered by a licensed healthcare provider and authorized through the student’s School Health Plan (SHP).
In accordance with the Utah Nurse Practice Act, the school nurse is responsible for assessing the student’s needs; developing the Individualized Health Plan (IHP) and SHP; determining appropriate delegation; and providing training, documentation requirements, and ongoing supervision consistent with Rule R156-31b-701a.
Designated personnel, acting in good faith and within the scope of the SHP and documented training, are not individually liable for civil damages, except in cases of gross negligence, willful misconduct, or actions outside authorized training or delegation.
1. HealthCare Provider Authorization Order for School Intermittent Catheterization Form
Before any catheterization is performed or assisted by trained school personnel, a completed and signed Cache County School District Catheterization Authorization Form must be on file. Authorization must be renewed annually, and updated with any change in the student’s ordered care.
2. Student School Health Plan (SHP)
Before any catheterization is performed or assisted by school personnel, a current SHP must be completed. The SHP is developed by the school nurse and signed by the parent/guardian and renewed annually for the current school year.
3. Individualized Health Plan (IHP)
The school nurse shall develop and maintain an IHP for students requiring catheterization. The IHP provides clinical oversight and evaluation and is based on the nurse’s assessment and the licensed health care provider’s order. The IHP functions in conjunction with the School Health Plan (SHP), which provides task-specific direction for trained and designated school personnel. The IHP is reviewed at least annually and as needed.
4. Personnel Training
Designated school personnel must be trained by the school nurse prior to performing or assisting with catheterization.
The school nurse(s) will provide all training and oversight for designated staff responsible for assisting with catheterization.
This service will be carried out in a safe, confidential, and respectful manner to support student health, dignity, and participation in school activities.
5. Definitions
Clean Intermittent Catheterization (CIC): Insertion of a catheter into the bladder to drain urine at prescribed intervals.
Delegation: Assignment by a licensed nurse to trained personnel to perform specific tasks under supervision consistent with Utah Nurse Practice Act requirements.
