Cache County School District is committed to maintaining an ethical, transparent, and accountable environment. The purpose of this reporting form is to notify Cache County School District (CCSD) of concerns specifically related to suspected fraud, financial misconduct, misuse of district resources, or violations of law, policy, or ethical standards.
This form is intended for district employees and community members to report suspected fraudulent or unethical activity. This form is not intended for general complaints, personnel disputes, student discipline concerns, or routine operational issues, which should be addressed through normal school or district communication channels.
